Our focus is to provide high-quality care for participants facilitated by teamworking between Registered Nurses of Haven Care, general practice, supported by regular team meetings, named clinicians and shared projects. Our services are available 24/7 throughout the year including public holidays. Our leadership team is always available to discuss your concerns or answer any questions about your/ participant’s care at any time. This service aims to enhance independence and health outcomes, helping people to stay at home through NDIS.
