Our outreach services help people who are living with mental illness and physical challenges to increase their independence to develop and re-establish daily living skills and self-care strategies, working towards you being able to integrate back into the community. Encouraging your choice and control, we connect across services and a range of services to support you to connect to your community.

Working in collaboration with Community Mental/Health teams, NDIS support coordinators and other providers, drug and alcohol teams, Aboriginal Health Service, as well as local GPs, psychologists and psychiatrists, we tailor our approach to help you achieve the best outcomes.

This will involve the provision of personal, social and domestic care, training and opportunities to engage in meaningful activities and experiences at home and within the local community. Our support workers will work closely with all members of the community team to enable their clients to continue living in the community with comfort and dignity. The overall aim is to improve the quality of life of people who have mental health difficulties or physical challenges and prevent inappropriate admissions to hospital.
